  Dr. Samuel Forlenza Jr.

  Doctor of Chiropractic

  Dr. Samuel Forlenza is a dedicated chiropractor with over 35 years of experience in treating patient care, focused in trauma and pain management. He currently serves as the Clinic Director of Ky Injury Care Centers, overseeing locations in Louisville and Erlanger, Kentucky. Since 2012, he has built a robust history of leadership in various chiropractic settings, including multidisciplinary pain management and community-based clinics. Dr. Sam Forlenza Jr. began his career in 1990 after earning his Doctor of Chiropractic degree from Life Chiropractic College in Georgia. He has held clinic director positions at several chiropractic centers, including Dry Ridge Chiropractic and County Chiropractic Center. His expertise extends to business development, reflecting his versatile background that includes a significant role in network sales before returning to chiropractic care. In addition to his clinical work, Dr. Forlenza has pursued extensive post-graduate education, focusing on advanced diagnostics and trauma care. His certifications include MRI Spine Interpretation, Neurodiagnostics, and various protocols related to the clinical management of injured & trauma patients. This commitment to continuous education enables him to provide comprehensive care, integrating innovative techniques and evidence-based practices. Dr. Sam Forlenza holds active chiropractic licenses in both Kentucky and Mississippi. He has a deep understanding of the complexities of spinal biomechanics and the impact of trauma, allowing him to deliver tailored treatment plans that address both acute and chronic pain. Outside of his clinical duties, Dr. Forlenza is married and a proud father to three children. His oldest son is a Physician in Residency, his daughter is currently in medical school, and his youngest is pursuing a nursing degree. In his spare time, he enjoys coaching baseball, reflecting his passion for mentoring and fostering teamwork in the community. With a focus on patient-centered care and a wealth of knowledge in chiropractic methodologies, Dr. Forlenza continues to make a significant impact on the lives of his patients, promoting recovery and enhancing quality of life.

  Dr. Jeffrey Shall, MD

  Board-certified Orthopaedic And Spine Surgeon

  Dr. Jeffrey Shall is a board-certified orthopaedic and spine surgeon. Dr. Shall focuses on treating a range of different conditions with spinal cord stimulator treatments. During his childhood, Dr. Shall worked on a horse farm where he developed his love for medicine. During his career, Dr. Shall has worked to become a leader in the medical field. He was one of the first surgeons to begin performing minimally invasive stabilization procedures. He specializes in fractures, dislocations, derangement, sprains of the spine, intervertebral disc degeneration and herniation, osteoarthritis, and spinal stenosis. Shall completed his residency training at Mt. Sinai Medical Center after graduating from The Ohio State University Medical School. The spine surgery fellowship with Dr. John Collils was completed in 1992. He is a fellow of the American Academy of Orthopedic Surgeons and a fellow of the North American Spine Society. In addition to pursuing excellence in his professional life, he enjoys spending time with his children and teaching them about his favorite hobbies such as motorcycling and skiing. His children are his greatest source of joy.
